Excellence in Diabetes Care

The diabetes program was created to enhance the education and support offered to our patients suffering with Diabetes.  We are wholeheartedly focused on improving the lives of our patients with Diabetes. Our Diabetes Program goals are to:

  • reduce the occurrence of serious illness that could result in hospital stays and ER visits
  • reduce diabetes-related complications, such as, blindness, amputations, kidney failure and heart disease
  • provide state-of-the-art  treatment, monitoring, and education  
  • promote self-management and a team approach to  Diabetes care
  • reduce your health care costs
  • And, most important to us: make your Life with Diabetes: Better and Longer

Our clinic is recently recognized by the NCQA and ADAhaving met national quality standards in diabetes care.

NCQA Goals and Attainment

Our diabetes educator, Lindsey Coffman, is a registered and licensed dietitian that is passionate about helping people with a variety of diabetes-related issues. She graduated from the American Dietetic Association-Accredited Coordinated Dietetics program at the University of Missouri-Columbia.  Lindsey is a great resource for your Diabetes-related education, questions and concerns. Lindsey is able to empower our patients with the knowledge to meet and exceed their Diabetes self care goals.  

Patient Care

Since our diabetes programs inception in 2005, an estimated 350 patients have completed our program.  Patients in the diabetes program work closely and meet regularly with our diabetes educator.  We have four available options for patients to receive education.   1. One on one education
  2. Group class education
  3. Bi-monthly education events
  4. Continuous glucose monitor system

Your individual needs and schedule will determine which option(s) is best suited for your Diabetes Care.
