Clay Platte Family Medicine Clinic is pleased to announce the addition of Dr. Salam, an accomplished Endocrinologist, to their practice. Dr. Salam is available for collaborations and patient referrals for diabetes and endocrinology care.
Dr. Salam has been a practicing physician in Missouri since 2009 and has served as an Associate Professor of Medicine at the University of Missouri-Columbia. He has extensive experience in teaching medical students and treating patients in the field of endocrinology with a focus on diabetes, thyroid, parathyroid, osteoporosis and hypogonadism. Dr. Salam has earned recognition as a Fellow of the American College of Endocrinology and the American College of Physicians. He serves as a member of a panel of experts for the Diabetes Outreach Program for the state of Missouri.

Our Diabetes Program was created to enhance the education and support offered to our patients suffering with diabetes. We are wholeheartedly focused on improving the lives of our patients with diabetes. Our Diabetes Program goals are to:
- reduce the occurrence of serious illness that could result in hospital stays and ER visits
- reduce diabetes-related complications, such as blindness, amputations, kidney failure and heart disease
- provide state-of-the-art treatment, monitoring and education
- promote self-management and a team approach to diabetes care
- reduce your health care costs
- and, most important to us: make your life with diabetes better and longer
Patient Care
Since our diabetes program’s inception in 2005, an estimated 350 patients have completed our program. Patients in the diabetes program work closely and meet regularly with our diabetes educator.
